在ICU气管插管复苏患者心理护理的临床效果观察_新生儿复苏时气管插管指征

　　【摘要】 目的 在ICU气管插管患者复苏过程即开始心理护理的临床效果观察。 方法 将符合入选条件的气管插管复苏患者60例随机分为对照组和观察组各30例，对照组实施ICU常规护理，观察组在此基础上于患者复苏开始实施心理护理。观察比较两组患者ICUS发生率、各项指标变化情况、撤机及撤机后再次插管情况。 结果 观察组ICUS发生率低于对照组，比较差异有统计学意义( P 0.05)，可能与例数太少有关。 结论 对ICU气管插管患者进行心理护理，对减少ICUS发生率有效，有利于患者康复。

　　In the ICU patients with trachea intubation recovery process, it started to the clinical effect of psychological nursing observation. Methods Will comply with conditions in the tracheal intubation 60 patients were randomly divided into recovery in control and the observation group each 30 cases and control implementation ICU conventional care, the observation group based on the recovery in patients began implementing the psychological care. Study the two groups were ICUS incidence, each index change situation, the ventilator weaning and pulled from the machine again after the situation. Results The observation group ICUS rate lower than those of the control group, more significantly ( P 0.05), may be too little and number relevant. Conclusion To the ICU patients with trachea intubation psychological nursing, to reduce the incidence ICUS effective, be helpful for patients recover.

　　重症监护室综合征 (intensive care unit syndrome，ICUS) 是指患者在ICU监护过程中出现的以精神障碍为主，兼有其他表现的一组临床综合征［1］。也称ICU精神障碍，ICU谵妄等。其发生率差异很大(2.7%～70%)［2］,分析不一致的原因考虑与研究对象的选择，精神症状的评价方法，手术前后有无并发症及其程度，药物应用的不同等因素有关。目前国内类似报道资料很少，我们对ICU气管插管复苏患者的心理护理研制了实施方案，自2008年1月开始实施，至2011年12月期间，我们对入住ICU气管插管患者复苏患者74例，符合本研究入选条件患者共60例，以住院号末位为双数的编入对照组共30例，单数的编入观察组共30例，开始实施心理护理，经临床观察，结果满意。现报告如下。
