动力加压钢板_锁定加压钢板和动力加压钢板治疗肱骨中下段骨折的临床疗效观察

　　[摘要] 目的 探讨锁定加压钢板和动力加压钢板治疗肱骨中下段骨折的临床疗效。 方法 选取我院2008年1月～2011年6月收治的肱骨中下段骨折患者82例，随机分为两组，采用动力加压钢板治疗的患者41例为对照组，采用锁定加压钢板治疗的患者41例为观察组，从术中出血量、手术时间、住院时间、骨折愈合时间、并发症、术后功能恢复情况等方面比较两组患者的临床疗效。 结果 观察组术中出血量[（204.8±36.5）mL]、手术时间[（56.4±10.9）min]、住院时间[（21.4±6.7）d]、骨折愈合时间[（10.5±2.1）周]均明显少于对照组[（253.7±48.1）mL、（79.6±14.3）min、（25.2±8.0）d、（12.8±3.4）周]，差异有统计学意义（P < 0.05）；观察组并发症发生率（4.9%）明显低于对照组（22.0%），观察组术后总优良率（95.1%）明显高于对照组（80.5%），差异均有统计学意义（P ＜ 0.05）。 结论 采用锁定加压钢板内固定治疗肱骨中下段骨折，对患者机体软组织损伤较小且固定牢靠，患者骨折愈合时间较早，利于患者进行早期的功能恢复性锻炼，大幅降低了患者并发症的发生率，是一种安全有效的手术治疗方法，值得临床推广使用。

　　Clinical curative effect observation of locking compression plate and dynamic compression plate in the treatment of middle and distal humerus fractures YAHG Xiaorong

　　[Abstract] Objective To investigate the clinical efficacy of locking compression plate and dynamic compression plate in the treatment of middle and distal humerus fractures. Methods 82 cases of patients with middle and distal humerus fractures in our hospital from January 2008 to June 2011 were selected and randomly divided into two groups. 41 cases of patients who were given dynamic compression plate treatment were selected as the control group. 41 cases of patients who were given locking compression plate treatment were selected as the observation group. Clinical efficacy of bleeding volume in operation, operative time, hospitalization time, fracture healing time, complications, postoperative functional recovery and other aspects were compared between the two groups. Results The bleeding volume in operation [(204.8±36.5) mL], operation time [(56.4±10.9) min], duration of hospital stay [(21.4±6.7) d], fracture healing time [(10.5±2.1) weeks] in the observation group were significantly lower or shorter than those of the control group [(253.7±48.1) mL, (79.6±14.3) min, (25.2±8.0) d, (12.8±3.4) weeks], the differences were statistically significant (P < 0.05). The incidence of complications in the observation group (4.9%) was significantly lower than that in the control group (22.0%); the excellent rate in the observation group (95.1%) was significantly higher than that in the control group (80.5%), the differences were statistically significant (P < 0.05). Conclusion Locking compression plate fixation in the treatment of middle and distal humerus fracture has small damage to the soft tissue and firm fixation, the fracture healing time is early, it is beneficial for patients to do early functional rehabilitation exercise, which can reduce the incidence of complications. That is a safe and effective operative method and worthy of clinical use.
